Good Faith Estimate Notice

Under the law, health care providers must give patients who do not have insurance or are not using insurance an estimate of the expected charges for medical items and services. This is called a Good Faith Estimate (GFE).

Your Rights Under the Law

You have the right to receive a Good Faith Estimate that outlines the total expected cost of any non-emergency items or services.

The estimate includes related costs such as:

  • Medical tests
  • Prescription drugs
  • Medical equipment
  • Facility fees or related charges

What You Can Expect

  • You will receive your Good Faith Estimate in writing at least one (1) business day before your scheduled medical service or item.
  • You can request a Good Faith Estimate from any healthcare provider, including East Bay Disc Centers, before scheduling a service.

If Your Bill Is Higher Than Expected

  • If you receive a bill that is $400 or more than your Good Faith Estimate, you have the right to dispute the bill.

  • Be sure to save a copy or photo of your Good Faith Estimate for your records.
